International Study Forms for Study Abroad Canada

In this lesson, we will go through the form completion process from the point of view of an Indian prospective student. Devendra Shankar has been accepted to the University of Waterloo for a Bachelor's of Applied Science in Mechanical Engineering program.

Official application forms have very specific rules as specified by the Canadian government. Print out the forms we have provided for download, and follow how Devendra would be filling them out:

Application for a Study Permit (also known as form IMM1294)

Follow the directions exactly as stated below, for each numbered item on the form.
  1. Print your name under the applicant section and provide the information requested for each of your family members. You must provide details about each of your family members and indicate whether they will be accompanying you to Canada. If you have more than three family members, photocopy the first page of the application before you start completing it. Make enough copies to fill in details about all your family members. Print all names as they appear on the passport or identity document. (Do not use initials). Make sure to read the Common Mistakes to Avoid page so you don't accidentally transpose first name and surname, etc. In this case, Devendra is coming by himself, so he doesn't have to worry about filling out information about dependents.
  2. Provide your current contact information (e.g.mailing address, telephone number, and fax number). All correspondence will be sent to this address.Note: If you wish to authorize a Canadian representative to receive correspondence concerning your application, indicate their address in this box and on the form provided in this page. Devendra has an uncle who lives in Canada (in Toronto, Ontario), who has agreed to act as his representative.
  3. Provide your residential address if different from your mailing address (for example, if you have a post office box where you receive mail, rather than at your home). In this case, Devendra is able to receive all his correspondence at home, and doesn't need to worry.
  4. Provide the name of your present job and describe your position as clearly as you can. Devendra is a student, so he skips this section.
  5. Print the name and address of your present employer or school. Devendra checks to make sure he's got the right spelling of the name and address of his home school.

    1. Provide details of your immigration status in the country in which you are applying- this may be temporary resident, student, worker, permanent resident or citizen. In this case, an Indian applicant like Devendra would say citizen.
    2. Print the expiry date of your temporary status in the country where you are making your application. (This is not applicable for a citizen or permanent resident of India.)
  6. Provide the full name and complete address of the educational institution you will be attending while in Canada. Attach an original letter of acceptance from the educational institution. 
  7. Provide the name of your course and its level. In Devendra's case, Bachelor of Applied Science in Mechanical Engineering.
  8. Provide the date your course in Canada will start and the date it will finish. Devendra's plan, for a four year program of study would mean 08/09/2008 until 30/04/2012.
  9. Provide the total cost of your education and living expenses while in Canada.
  10. Provide details of funds available to you and your family members while in Canada. Indicate who will be paying your expenses. If you will be receiving any scholarships, bursaries or grants, place an “X” in the box “Other” and give details. Attach proof of funds available such as a bank
    statement, or a letter from the bank stating how much money you have in your bank account. The
    letter must indicate your name and account number.
  11. Each question in this section must be answered for each applicant. Place an “X” in the yes or no
    box. If you answer "yes" to any of the questions from 12 c) to g), give the name of the person with
    an explanation in the space marked "related information". Again,  Devendra is a single student coming to Canada by himself, so this section is left blank for him.
  12. Print an “X” in the box which applies to you or any accompanying family member. Provide the
    name of the countries and length of stay for each accompanying member.
  13. You must sign and date the application. It is very important that you do so, to avoid having the application returned to you. Devendra has signed and dated his application, and checked for completion.

Use of a Representative (also known as form IMM 5476)

Complete this form if you are appointing a representative.
Devendra's representative is his uncle Hassan Subramanyan, an unpaid representative. He will be checking to make sure Devendra's application is going smoothly, taking advantage of the fact that he lives in Toronto.
